A septorhinoplasty is a combined surgical procedure that addresses both the internal nasal septum and the external nose structure in a single operation. Unlike a standard cosmetic rhinoplasty focused solely on appearance, or a purely functional septoplasty aimed at breathing improvement, septorhinoplasty tackles both concerns simultaneously. This integrated approach has become increasingly common as surgeons and patients recognize that nasal form and function are inseparable.

The nasal septum is the wall of cartilage and bone dividing the left and right nasal passages. A deviated septum occurs when this partition shifts to one side, potentially obstructing airflow and causing breathing difficulty. When a patient presents with both a deviated septum and cosmetic concerns about nasal shape, size, or proportion, a single coordinated surgery can be more efficient and cost effective than staging two separate procedures. The cost of septorhinoplasty typically ranges from 8,000 to 18,000 dollars, though this varies by surgeon experience, geographic location, and complexity.

During septorhinoplasty, the surgeon accesses the septum through internal incisions, corrects the deviation by straightening or repositioning cartilage and bone, and simultaneously reshapes external nasal structures. The external work might involve reducing dorsal height, refining the tip, narrowing the bridge, or adjusting the overall proportions. Many surgeons use an open approach, which provides direct visualization of both internal and external anatomy, reducing the risk of missing structural problems that could compromise the final result.

One key advantage of the combined procedure is that addressing the septum often improves breathing immediately, while the cosmetic changes unfold over months as swelling resolves. However, this also means the surgeon must balance competing priorities: septal correction requires certain angles and positions that might not be identical to what pure cosmetic optimization would dictate. The recovery timeline for septorhinoplasty is similar to cosmetic rhinoplasty alone. Most patients can return to light activities within 7 to 10 days, though contact sports and heavy lifting should be avoided for 4 to 6 weeks. Nasal packing, if used, is typically removed after 24 to 48 hours. The visible swelling diminishes significantly within 2 to 3 weeks, but the nose continues to refine for up to 12 months as deeper tissues settle.

Complicaitons are rare but possible. Over correction of the septum can occur, occasionally requiring revision. Under correction, where the deviation persists or breathing does not improve as expected, may also require further surgery. There is a small risk of perforation in the septum itself, which can develop into a chronic issue if not addressed promptly. Aesthetic results might not align with preoperative expectations due to individual healing patterns or surgeon interpretation of the desired shape.

Preoperative consultation is critical. The surgeon should perform nasal endoscopy to visualize the extent of septal deviation and any other internal obstructions. Imaging such as CT scanning is less often used for routine cases but can help in complex revision scenarios. The patient should clearly communicate cosmetic goals while the surgeon explains what is anatomically feasible and how septal correction will influence the overall shape.
