Preservation vs. Structural Rhinoplasty: What Actually Changes Under the Skin

Two schools of nasal surgery promise natural results, but they get there by very different mechanical routes. Here is a plain-English comparison of what each technique removes, keeps, and rebuilds.

Ask three rhinoplasty surgeons how to lower a nasal hump and you may get two different philosophies. The first, often called structural rhinoplasty, removes or reshapes the hump directly, then rebuilds support with cartilage grafts. The second, preservation rhinoplasty, leaves the top surface of the nose intact and instead lowers the entire dorsal framework from underneath, like lowering a tent by shortening its poles rather than cutting the canvas. Both approaches are legitimate, both have decades of published technique behind them, and neither is universally better. What matters is the mechanics, and which mechanics fit a given nose.

How structural rhinoplasty works. In the conventional approach, the surgeon exposes the dorsum, shaves or rasps down the bony portion of the hump, and trims the cartilaginous portion. This opens the roof of the nose, so the sidewalls must be brought back together with controlled bone cuts called osteotomies. Because removing dorsal cartilage can weaken the middle vault, surgeons frequently place spreader grafts, thin strips of cartilage set between the septum and the upper lateral cartilages, to hold the internal nasal valve open and keep the bridge lines straight. The logic is subtractive first, reconstructive second: take away the excess, then reinforce what remains so it resists scar contracture over years.

How preservation rhinoplasty works. Preservation techniques treat the dorsum as a unit worth keeping. Instead of opening the roof, the surgeon removes strips of septal cartilage and bone beneath the dorsum, or performs impaction maneuvers on the bony pyramid, allowing the entire intact dorsal surface to drop and flatten into a lower position. Because the natural junction between bone and cartilage, the keystone area, is never disrupted, the smooth transition along the bridge is retained. In principle this reduces the risk of visible irregularities, an open roof deformity, or an inverted V shadow, complications that structural surgery specifically uses grafts to prevent.

Where preservation shines. Noses with a modest, smooth hump, straight bridge lines, and no significant deviation are the classic candidates. The dorsal skin and framework are already aesthetically acceptable in profile shape, just too high. Lowering the whole unit preserves natural dorsal aesthetic lines that are difficult to recreate with grafts. Some surgeons also report less need for camouflage material and, in select patients, faster settling of the bridge because the surface envelope was never violated.

Where preservation struggles. A crooked nose is a genuine problem for pure preservation, because lowering an intact but deviated dorsum simply lowers a deviated dorsum. Wide, convex, or S-shaped humps, thick kyphotic bony caps, and significant septal deviation often require conversion to structural maneuvers or hybrid techniques. There is also a recognized issue of hump recurrence: if the tissues at the keystone are not adequately released, the preserved dorsum can spring partially back toward its original height over months. Published revision discussions in the preservation literature focus heavily on this exact failure mode.

Where structural rhinoplasty shines. It is the more versatile toolkit. Severe deviation, prior surgery, trauma, major tip work, and breathing reconstruction all favor an approach where the surgeon can disassemble, straighten, and rebuild with grafts. Cartilage grafting also lets the surgeon set long-term support deliberately rather than relying on native anatomy, which matters because scar tissue contracts for years and pulls on whatever framework is left behind.

Where structural rhinoplasty struggles. Every graft is a judgment call. Overresection without adequate reinforcement historically produced the pinched, scooped, collapsed results that gave rhinoplasty a bad reputation in earlier decades. Modern structural surgery largely solved this with routine spreader grafts and stronger tip support, but the approach still depends on rebuilding contours by hand, and irregularities under thin skin remain a known revision driver.

The myth worth retiring. Preservation rhinoplasty is sometimes marketed as gentler, scarless, or inherently safer. Mechanically, that is not accurate. Preservation still involves removing septal tissue, mobilizing the bony pyramid, and in many versions performing multiple osteotomies. It preserves the dorsal surface, not the patient from surgery. Swelling, splinting, and recovery timelines are broadly similar, and outcomes depend far more on candidate selection and surgeon experience with the specific technique than on the label.

What patients should actually ask. Useful consultation questions include: Is my hump shape and septum suited to preservation, or would you convert to structural methods intraoperatively? How do you prevent hump recurrence at the keystone? If grafts are needed, where will the cartilage come from? A surgeon fluent in both philosophies can explain why one fits your anatomy, and many now blend the two, preserving the dorsum where possible and adding structure where needed. That hybrid reality, more than any branding, is where the field is heading.
